A pharmacist has been censured and fined by the Health Practitioners Disciplinary Tribunal after it found that she had practiced for 8 months after her practicing certificate expired and that she had provided misleading information to the Pharmacy Council.

The pharmacist had overlooked renewing her certificate and had not seen emails sent to her as reminders. She had completed a return to practice form on line but had provided incorrect information in the form. The Tribunal found that providing the incorrect information was a breach of the Code of Ethics.

As well as being censured she was fined $2000 and ordered to pay $17,000 costs.
