A company’s worker became trapped in machinery, suffering severe injuries causing his death.  The company pleaded guilty to failing to provide a safe workplace, and not providing adequate supervision and training.

This tragic incident led to a record penalty being imposed of a $10,000 fine and $195,000 reparation to the family.

An investigation found the company had not taken all reasonable steps to prevent the accident.  The company should have taken steps to provide a platform for workers to stand on while cleaning the machinery and they had failed to fit an alarm despite a recommendation following an earlier incident.

You must take all practicable steps to identify, and eliminate or isolate, hazards in the workplace.  Failure to do so can lead to prosecutions and large financial penalties.
