The Health Practitioners Disciplinary Tribunal has struck a nurse off the register after she changed notes following the death of a patient.  As well as preparing the notes which did not exist at the time of the patient’s death, the nurse also destroyed the existing notes and failed to report the falsification of notes by other staff members.

The Tribunal decided that as a matter of deterrence (not only for this nurse but also for members of the profession) that the message be given loudly and clearly, that behaviour of this kind cannot be tolerated.  Suspension in this circumstance would not be sufficient and striking off was necessary.
