An employee was tragically killed, while operating a hydraulic ram, when he was hit in the chest by high pressure hydraulic oil.

The employer needed to ensure that the ram was assessed and rated for the safe operating pressure.  In addition better training in the safe operation of the equipment was needed.

The employer was fined $52,500 and ordered to pay $100,000 in reparation.
