The Health Practitioners Disciplinary Tribunal has imposed conditions on a chiropractor who over nine years failed to keep any or adequate records on patients in relation to their treatment.  This involved over 7,000 patients where no records were kept and others whose records were inadequate.

The Tribunal ordered that the chiropractor be supervised for 18 months with the supervisor providing three monthly reports, with the focus on the adequacy of note keeping.  The practitioner is also to complete appropriate courses of education in relation to note keeping.  The practitioner was also ordered to pay $20,000 costs for the investigation and hearing.
