The Aged Care Commissioner has found that there were systemic failures of care and failures to follow policies and procedures at an aged care facility.

The facility failed to ensure that the resident was correctly assessed upon admission.  There was no documentation of assessment said to have been done and documentation of only 1 of 13 assessments that should have been done.

Wound care needs were not documented, and appropriate wound care was not done.  Pain relief was not planned or documented, which led to patient discomfort.

No appropriate first aid was given when the patient suffered a burn in the facility. Contingency plans for staff shortages were not put in place, and the patient suffered a burn to her leg.  A fall assessment and prevention plan was not put in place.

Staff lacked awareness to identify hazards. 

The patient’s bed was placed against a wall heater with the risk of injury and fire. 

Several individual nurses were found to have contributed to the situation and one failed to give appropriate first aid following the burn.

The aged care facility has been told to fix its processes for assessment and staff training and has been referred for possible proceedings against it due to the multiple systemic failures and has to apologise to the patient’s family for its failures.
