The Health Practitioners Disciplinary Tribunal has put conditions on the practice of a physiotherapist for two years after the physiotherapist failed to keep adequate notes on 200 occasions and also misplaced over 100 patient files which contained personal and health information.  The physiotherapist also failed to respond in a professional manner to the Physiotherapy Board and practiced without an annual practising certificate for a two week period.

The conditions imposed on the physiotherapist’s practice were designed to ensure that the physiotherapist remained diligent in the maintenance of her patient files.
