The Aged Care Commissioner has found a care facility in breach of the patient code of rights when a patient was administered medicine patches daily instead of every three days due to a prescription error by the GP.

The nursing staff should have been aware that daily application of the medicine patches was not normal and raised this with the GP.

In addition, nurses failed to remove the previous patches when applying the new ones, leading to an even greater overdose for the patient

The care facility has been ordered to apologise and provide proof of its amended training for staff and the registered nurse in charge has been ordered to reflect on her practice and the standard of her documentation which was unclear.
